File a Death Claim
The value of insurance is helping people in their greatest time of need, and we are sorry for your loss. We understand that losing a loved one can be an extremely difficult and overwhelming time, and our goal is to make the claims process as quick and easy as possible.
What you’ll need to file a death claim
- Insured’s full name
- Policy number(s), if available (not required)
- Insured’s date of birth
- Date of death
- Cause of death
- Location of death
- Information about individual(s) expecting to file a claim – name, mailing address, phone number
- Name of funeral home, if selected (not required)
Two ways to report a death
01. Online
Complete and submit the Notice of Death form.
02. By phone
Call (800) 233-3750, Monday-Friday, 8:00 AM to 3:00 PM CT, to notify us of the insured’s passing. If you’re assisting someone else with their claim, please let us know during the claim intake process.
What to expect
After we receive a notice of death, we will verify policy details and beneficiary information before sending out claim packets to confirmed beneficiaries. Our goal is to process and pay every claim as quickly as possible. If you are a confirmed beneficiary, you will receive a letter addressed to you outlining our requirements and submission instructions. Once all requirements are received, we aim to process and pay all claims within 10 business days.
Tips for success
- Check your forms for completeness and accuracy before submitting.
- Sign every form by hand. We require a wet signature and cannot accept electronic signatures.
- Review the letter we send you for any special instructions or additional requirements, and check the “Additional Requirements” checklist on the claim form itself.
- Scan your documents at high quality if submitting by email or fax. We do not accept photos of documents.
We may require the original certified death certificate mailed to our office in some cases.
Required documentation
Requirements vary by policy and situation. As a common standard, however, we require the following:
- Claim form from all named beneficiaries
- Certified original copy of death certificate
This is not an exhaustive list of claim requirements. Depending on the circumstances of your claim, requirements and supporting documentation may vary.
